***JOB OPENING*** Trinity Institution - Homer Perkins Center, Inc. Job Description POSITION TITLE: Clinician RESPONSIBLE TO: Director of Prevention POSITION DESCRIPTION: To work as part of a clinical team providing in home individual and family counseling to prevent child abuse and neglect and/or out of home placement and promote positive relationships for youth 0 - 18 and their families. ACTIVITIES, TASKS AND ASSIGNMENTS: o Establish and maintain professional relationships with consumers, service providers, funding sources and other staff o Conduct formal assessments focusing on consumer strengths, goals and outcomes using a solution oriented/trauma informed approach o Provide in house individual and family counseling as needed o As part of a team, assist with case management, crisis intervention and make referrals o Conduct weekly home visits o Schedule and participate in treatment review and other service provider meetings o Complete service plans with clients o Participate in staff development opportunities, in-service trainings and contribute to the program's continuous improvement o Encourage parent participation in program and agency activities o Ensure timely completion and submission of all materials to bring records into compliance with internal and external agency regulations o Flexible work hours to accommodate families REQUIRED KNOWLEDGE, ABILITIES AND SKILLS: o MSW or MA in related field and a minimum of three (3) years of clinical experience

Family Support Worker- Prevention

POSITION DESCRIPTION:
To prevent child abuse and neglect and/or out of home placement and promote positive relationships for youth ages 0-18 and their families.

ACTIVITIES, TASKS AND ASSIGNMENTS:
• Establish and maintain professional relationships with clients, service providers, funding sources and other staff
• Provide case management, crisis intervention and make referrals
• Conduct weekly home visits
• Schedule and participate in treatment review and other service provider meetings
• Complete service plans with clients
• Supervise visits with children and caregivers, and provide verbal or written reports to the Prevention Director and court system or funding source
• Participate in staff development opportunities, in-service trainings and contribute to the program’s continuous improvement
• Encourage parent participation in program and agency activities
• Ensure timely completion and submission of all required paperwork
• Flexible work hours to accommodate families
• Additional functions as determined by the Prevention Director

REQUIRED KNOWLEDGE, ABILITIES AND SKILLS:
  • AA, BA preferred and minimum of one year direct experience working with children and families
